SUMMARY:The 20th Annual TCOM Conference
DESCRIPTION:The Center for Innovation in Population Health at the Universit
 y of Kentucky\, in collaboration with the Praed Foundation\, is organizing
  the 20th Annual Transformational Collaborative Outcomes Management (TCOM)
  Conference. This event will feature presentations showcasing the applicat
 ion of the TCOM framework and its associated tools.\n\nTCOM serves as the 
 foundational model for various tools such as ANSA\, CANS\, FAST\, SSIT\, C
 AT\, and others\, designed to support systems\, organizations\, and progra
 ms in their mission to facilitate meaningful life changes for individuals.
  Attendees can expect to explore how TCOM is used to improve organizationa
 l outcomes and individual transformation.\n\nProgram and Accreditation Inf
 ormation Details regarding the program schedule and accreditation will be 
 shared soon.\n\nLearning Objectives for the Conference By the end of the c
 onference\, participants will have gained the ability to:\n\n 	Discuss eff
 ective communication strategies with individuals\, families\, and professi
 onals using a unified language.\n 	Analyze how the TCOM framework helps cr
 eate a shared vision between clients\, service providers\, and systems.\n
